Medical Checklist

Fill the Checklist before book a scuba diving

  • A medical certificate is not required:
    – If you answer NO to the first 10 questions.
    – If you answer YES to questions 1, 2, 4, 6, 7, 8 and/or 9, and NO to the corresponding questions on page 2.

  • A medical certificate is required:
    – If you answer YES to questions 3, 5 and/or 10.
    – If you answer YES to questions 1, 2, 4, 6, 7, 8, 9 and/or 10 and also YES to any of the corresponding questions on page 2.

  • If you already have a medical certificate less than one year old authorizing you to dive:
    – You do not need to read or sign the medical questionnaire.

  • All training divers, introductory divers, and certified divers on exploration/fun dives must sign this questionnaire before diving.

  • If any of the questions apply to you, please print the pages following the questionnaire and present them to your doctor or a doctor specialized in diving medecine, so they can authorize or deny your participation in scuba diving.
